By checking the box below, I acknowledge and authorize Excelsior University to request and obtain my official academic transcript and related supporting information from Lake Forest Graduate School of Management on my behalf. This authorization made pursuant to the Family Educational Rights and Privacy Act (FERPA) applies to my Lake Forest transcript, which is required for admission and transfer credit evaluation into Excelsior University’s master’s level degree program. If I ever wish to withdraw this authorization, I may do so by contacting the Excelsior University Registrar.

Note: You are transferring to a specific term. Please keep in mind that once admitted, you must register for an approved course that begins within the first three consecutive terms from your official admission date and remain in your course past its cancellation period (first 15 days after the course’s start date) in order to be considered enrolled. Otherwise, your admission to the university expires. If you fail to register for a course, you will be required to reapply and go through this process again. If you need to delay your start, please contact your admissions counselor.

By clicking Submit, you are taking an important next step in earning your degree. Upon receipt of this transfer form and your official transript, we will begin your transfer review. Upon its completion, your primary advisor will be in touch with you to answer any questions you may have and help you enroll.

To apply for federal financial aid, follow this link to transfer to the United States Department of Education’s Free Application for Federal Student Aid (FAFSA). The Excelsior University code is 014251. Entering this on your FAFSA will ensure Excelsior University receives the results.

Submitting this form is the first step in the transfer process. When admitted to the University, you will receive two emails from Excelsior University that will allow you to create a MyExcelsior account— one asks you to verify your information entered and create a password, and the second provides your username. You will use this username and password to log into your MyExcelsior account to access your transfer review after our evaluators completed your review.
